We are seeking a skilled Nurse Practitioner specializing in Hospice/Palliative Medicine for a 40-day locum tenens assignment in Maryland, starting May 12, 2025. The ideal candidate will provide comprehensive care for in-home hospice and palliative care patients across two counties. This role combines in-person and virtual visits, with clinic responsibilities 2-3 days a week. Key duties include medication management, symptom control, and pain management for patients with various illnesses. The work schedule is Monday to Friday, 8:00 AM to 5:00 PM. Proficiency with Netsmart EMR is required. This opportunity offers the potential for extension beyond the initial assignment period.

Job Responsibilities and Duties:


• Conduct initial in-person visits for hospice and palliative care patients


• Perform follow-up virtual visits to ensure continuity of care


• Manage medications, symptoms, and pain for patients with various illnesses


• Provide comprehensive care in both home and clinic settings


• Collaborate with interdisciplinary teams to develop and implement care plans


• Accurately document patient encounters using Netsmart EMR


• Demonstrate strong communication skills with patients, families, and healthcare team members


• Maintain a patient-centered approach to end-of-life and palliative care


• Adapt to the unique needs of patients across two counties


• Stay current with best practices in hospice and palliative medicine


Additional Information:


• Must be licensed as a Nurse Practitioner in Maryland


• Experience in hospice and palliative care required


• Proficiency with Netsmart EMR system preferred


• Strong organizational skills and attention to detail essential


• Ability to work independently and as part of a team
