Job Opportunity: Nurse Practitioner - Palliative Care

Job Responsibilities:
- 75% Inpatient Palliative Care; 25% Outpatient Palliative Care
- Deliver comprehensive Inpatient and Outpatient Palliative Care services
- Conduct patient consultations for symptom management and advanced care planning
- Lead discussions with patients and families about terminal diagnoses and care goals
- Perform inpatient rounds, including ICU rounds, for palliative and hospice patients
- Manage outpatient palliative visits through various facilities, including Telehealth
- Collaborate with hospital specialists to align care plans
- Provide independent care in a fast-paced hospital environment
- Maintain thorough documentation in Meditech and eClinicalWorks
- Serve as a clinical expert in palliative care, educating staff and advocating for patient-centered care
- Manage up to 6 patient encounters per day over a 5-day workweek
Qualifications:
- Board Certified Nurse Practitioner (REQUIRED)
- Valid Ohio license or willingness to obtain (REQUIRED)
- Experience as an advanced provider in an acute care setting or with palliative and hospice care (REQUIRED)
- Clean malpractice record and an active, valid license (REQUIRED)
- Strong desire to work in a nonprofit/community-based setting
License and Certification Requirements:
- Board Certified NP (REQUIRED)
- License to practice in Ohio (or willing to obtain) (REQUIRED)
