Position: Hospice House RN

Location: Benton Franklin County

Type: Part Time

Anticipated Schedule: Two 12 hour night shifts per week

Position Overview:

The purpose of a Hospice Registered Nurse (RN) job description is to provide compassionate and comprehensive care to terminally ill patients and their families in a Hospice House setting. The RN, under the supervision of the Hospice House Clinical Nursing Supervisor, plays a vital role in identifying patient and family needs and coordinating services to deliver preventive and supportive care. The RN also oversees the work of LPNs, NACs, and volunteers in the Hospice House. Additionally, the RN demonstrates a commitment to the philosophy, goals, and objectives of hospice care by utilizing teaching, assessment, and intervention skills to enhance the quality of life for patients and families.

Minimum Qualifications:

1. Current Washington State license as a professional registered nurse.

2. At least two years of nursing experience, preferably with one-year acute care, home health, or hospice nursing.

3. Availability for day, night, and weekend shifts as needed and as scheduled by the supervisor.

4. Available to work a minimum of two holidays per year.

CLINICAL STAFF POSITION QUALIFICATIONS

1. As required per position/credentialing; successful completion of Blood Born Pathogens Training

2. (Hospice House) Washington State Food Worker Card issued by the Department of Health

RESPONSIBILITIES AND ESSENTIAL FUNCTIONS

1. Accept responsibility for the care of patients admitted to or transferred into the Hospice House setting. A. Conduct an initial assessment of patient and family needs, including physical, emotional, social, and spiritual factors.

B. Initiate and update the individualized plan of care in collaboration with the IDT.

C. Accomplish nursing assessments of all the unit's patients during the shift according to policy.

D. Administer medications and treatments to patients.

2. Request additional assessments and services from other disciplines on the team as needed.

3. Assess patient/family knowledge of disease and care and determine deficiencies in that knowledge.

4. Evaluate the patient/family's ability to learn and follow instructions.

5. Provide education, instruction, and information as needed.

6. Provide direct nursing care to patients to achieve the highest level of comfort, continuity of care, and quality of life.

7. Observe signs and symptoms, reactions to treatments, including drugs, and report changes in the patient's physical or emotional condition to the physician.

8. Provide emotional support to patients and families in a nonjudgmental atmosphere.

9. Affirm patient/family lifestyle and involve them in decision-making regarding care.

10. Coordinate care with other interdisciplinary team members and community resources. Provide continuity of care when the patient is transferred back into the home setting.

11. Maintain complete, accurate, current patient records in accordance with Hospice record-keeping procedures and requirements.

12. Participate in the shift-to-shift reports, monitor and report changes in the patient's condition or family situation, and participate in assessing the need for changes in the plan of care.

13. Inform other IDT members involved in the patient's care of changes and needs as they occur during their Hospice House stay.

14. Assess the patient acuity using the Acuity Tool each shift and communicate needs for staffing/low census with the supervisor or administrator on call.

15. Complete cross-training in all nursing departments.

16. Float to other areas or take low census hours as necessary and directed

17. Practice resource stewardship when utilizing supplies, medications, DME, and staffing resources.

18. Supervise and direct care of Hospice House LPNs, NAC, and volunteers scheduled on your shift.

19. Assess the LPNs and NAC's knowledge of disease and care needs of individual patients and provide education, instruction, and information as needed.

20. Assist the Clinical Supervisor in the evaluation of the NAC's skills and care provided to patients, as requested.

21. Communicate with the Clinical Supervisor any problems, changes, or issues that may arise in providing appropriate quality care to any patient in the Hospice House.

22. Communicate any changes or problems in the patient's condition, or family situation, with the physician on a regular basis through telephone reports or during the physician's visit.

23. Follow the Hospice House Bereavement Policy in collaboration with the Bereavement Coordinator.

24. May be appointed to act in the absence of the Clinical Supervisor.

25. Attend regular Hospice House staff meetings and care conference meetings.

26. Maintain knowledge of currently applicable regulations and program policies/procedures pertaining to patient care and performance of nursing responsibilities.

27. Maintain and enhance nursing skills and knowledge through formal education, workshops and conferences, active participation in professional and related organizations, and individual research and reading.

28. Contribute to agency operation and growth by participating in staff meetings, policy/procedure development, planning and evaluating services, and performance improvement activities.

29. Participate in training new staff and volunteers.

30. Participate in education and public relations activities within the community as requested by the Chief Clinical Officer or the Chief Executive Officer.

31. Prepare and submit reports on patient services and other daily activities as required.

32. Perform other duties as assigned.

Compensation

Starting Range: $35.50 - $55.00 per hour

Max Range: $35.50 - $73.81 per hour

These ranges are tailored to match the diverse programs and services we offer. Our goal is to ensure fair compensation for candidates based on these factors while maintaining internal peer equity.

Benefits

Mileage Reimbursement: $0.67 per mile, for eligible positions.

Sick Time: Earned on an accrual basis, with a 90-day hold for new hires (usable after 90 days)

3. Responsible for knowledge and compliance with agency policies, procedures, and practices.

4. Responsible for maintaining and protecting Patient and agency information in a confidential manner. Practice appropriate HIPAA confidentiality by accessing patient-related information on a "need to know" basis only.

5. Reliable personal transportation, and current and valid driver's license and vehicle insurance.

6. Satisfaction of state and agency requirements for Tuberculin screening.

7. Hepatitis B immunization series, proof of series, or signed declination.

8. Proof of applicable Vaccination(s) on file or approved accommodation/declination as appropriate on file.

9. Must pass a criminal background screening and pre-employment drug screening.

10. Ability to meet the physical demands of the position (see attached physical requirements and environmental conditions).

11. Complete agency and departmental orientation and training program(s).

12. Complete all continued learning, education, policy reviews, and required training by their respective deadlines.

13. Maintain licensure(s) and certification(s) required for the position.

14. Available to work as assigned/scheduled.
