As a Hospice Clinical Supervisor, here's a big-picture view of what you'll do:

  • Provide thoughtful leadership and support for all staff and patient care services, including making appropriate caregiver assignments, ensuring seamless coordination of care, and overseeing the ongoing development and updates of personalized care plans

  • Foster open, compassionate communication with all members of the interdisciplinary team to ensure each patient's care is cohesive and centered on their unique needs and wishes

  • Support continuous, collaborative assessment of each patient's condition to ensure comfort, dignity, and quality of life

  • Guide the development of educational resources and ensure clinical information is shared with patients, families, and caregivers in a clear and supportive manner

  • Contribute meaningfully to our Quality Assessment and Performance Improvement (QAPI) Program, helping us continually enhance the care and comfort we provide

A few must-haves for Hospice Clinical Supervisor:

  • A degree from an accredited nursing program and an active RN license in the state of Louisiana

  • 3-5 years of meaningful leadership experience in hospice or home health, guiding care with empathy and expertise

  • Current CPR certification, along with a valid driver's license, dependable transportation, and up-to-date auto insurance-so you can safely reach those who need you most

  • Willingness to provide support with leadership and on-call responsibilities, ensuring continuity and peace of mind for patients and staff

  • A strong understanding of both state and federal regulations for home health and hospice, upholding the highest standards of care

  • Clinical skill, excellent communication and organizational abilities, and-above all-a heart full of compassion for those navigating life's most tender moments
