What You Will Do as a Hospice Clinical Manager RN

As the Hospice Clinical Manager, you will be responsible for the day-to-day operational oversight and management of the hospice interdisciplinary team. You will be responsible for clinical oversight, implementation and ongoing assessment of the hospice plan of care, patient advocacy, monitoring patient/family satisfaction, data collection, reporting, quality initiatives, compliance, and ongoing education of the hospice team. Some of your primary responsibilities include:

  • Auditing charts for clinical oversight and compliance with the federal and state regulations
  • Managing clinical activities, including patient assessments, care plan development, service level determination, on-site field visits, and implementation and evaluation of care plans
  • Participating in recruiting, interviewing, selecting, and orienting team members. Providing direct supervision and evaluation of the hospice team
  • Ensuring service quality and participating in care coordination to ensure proper communication between caregivers, patients, referral sources, and payers
  • Maintaining financial responsibility related budget goals through ensuring documentation supports frequency and duration of goals, providing appropriate and accurate patient care, accurate diagnosis coding, and appropriate delivery of patient care
  • Ensuring the comprehensive assessment reflects a thorough evaluation of the patient's physical, psychological, emotional, and spiritual status related to the terminal illness and related conditions
  • Providing patients with a safe and comfortable environment

Education, Licensure & Certification Requirements

  • Active RN licensure
  • Minimum of three (3) years of hospice or home care experience
  • Minimum of one (1) year in a clinical management role
