The Registered Nurse provides skilled, intermittent care in patients' homes, following physician s orders and care plans under supervisor guidance.
•  Responsibilities include evaluating and updating patient care, administering specialized nursing procedures, coordinating services, and educating patients and families.
•  Prepares documentation, communicates patient changes, and collaborates with multidisciplinary teams.
•  Qualifications include licensure, a minimum of two years experience preferred, and ability to exercise judgment independently.
•  Work environment involves travel to patient homes, with physical requirements such as lifting and extended standing.
•  Supervised by the Clinical Manager, with oversight of LPNs and aides.
