One of my top clients is seeking a compassionate and skilled Home Health Physical Therapist to provide exceptional care to our geriatric patients throughout Sacramento, California. Responsibilities:


  • Conduct comprehensive physical therapy assessments and develop individualized treatment plans for geriatric patients in their homes and/or skilled nursing facilities (SNFs).
  • Implement therapeutic exercises, manual therapy, and other interventions to improve mobility, strength, balance, and functional independence.
  • Educate patients and their families on proper techniques for exercise, fall prevention, and home safety.
  • Maintain accurate and timely documentation of patient progress and treatment plans.
  • Collaborate with other members of the healthcare team, including physicians, nurses, and occupational therapists.
  • Travel to patient homes and SNFs within the designated service area.

Qualifications:


  • Valid California Physical Therapist (PT) license.
  • Minimum Specify Number years of experience working with geriatric patients.
  • Proven expertise in geriatric physical therapy assessment and treatment techniques.
  • Strong understanding of common geriatric conditions and related physical limitations.
  • Excellent communication, interpersonal, and patient education skills.
  • Ability to work independently and manage a flexible schedule.
  • Valid driver's license and reliable transportation.
