Your Impact as a Home Health Physical Therapist: - Provide one-on-one care in clients' homes, ensuring they receive top-quality therapy.
- Perform mobility and functional assessments, develop individualized care plans, and help clients regain strength and independence.
- Educate clients and their families on exercises, safety measures, and strategies to enhance daily living.
- Collaborate with a multidisciplinary team to deliver holistic, patient-centered care.
- Document progress and care plans efficiently using state-of-the-art EMR technology.
What We're Looking For: - Active NC Physical Therapist (PT) license
- Graduation from an accredited PT program (APTA or CAHEA/AMA-approved)
- Strong time management, communication, and problem-solving skills
- Ability to work independently while staying connected to a supportive team
