Schedule: Full-Time Home Health Visits

What You'll Do: - Visit clients in their homes within your local territory (no facility-based assignments).
- Perform comprehensive evaluations of functional status, including strength, coordination, endurance, and ADLs/IADLs.
- Design and implement individualized intervention plans with a focus on safety, independence, and quality of life.
- Assess home environments for safety hazards and recommend equipment or modifications.
- Educate clients and their caregivers on strategies to support rehabilitation and prevent complications.
- Collaborate with the interdisciplinary team and adjust care plans as needed.
- Document electronically using a user-friendly tablet and EMR system.
What You'll Need: - A current Occupational Therapist license in Vermont.
- At least 1 year of recent OT experience (home health experience preferred but not required).
- Graduation from an accredited Occupational Therapy program.
- Strong time management, communication, and critical thinking skills.
- Comfort working independently in the field and using digital documentation.
