Location: South Burlington Visits Office
Territory: Washington County Adult & Geriatric Clients
Schedule: Full-Time Home Health Visits

What You'll Do:
  • Visit clients in their homes within your local territory (no facility-based assignments).
  • Perform comprehensive evaluations of functional status, including strength, coordination, endurance, and ADLs/IADLs.
  • Design and implement individualized intervention plans with a focus on safety, independence, and quality of life.
  • Assess home environments for safety hazards and recommend equipment or modifications.
  • Educate clients and their caregivers on strategies to support rehabilitation and prevent complications.
  • Collaborate with the interdisciplinary team and adjust care plans as needed.
  • Document electronically using a user-friendly tablet and EMR system.
What You'll Need:
  • A current Occupational Therapist license in Vermont.
  • At least 1 year of recent OT experience (home health experience preferred but not required).
  • Graduation from an accredited Occupational Therapy program.
  • Strong time management, communication, and critical thinking skills.
  • Comfort working independently in the field and using digital documentation.
