Registered Nurse (Home Health) - $100K-$110K Annual Salary.

A leading home health organization is currently seeking a skilled and compassionate Registered Nurse to provide in-home patient care. In this role, you will manage all aspects of patient care, ensuring high-quality clinical services within a supportive and collaborative environment. This is a full-time , Monday-to-Friday position with competitive salaries-higher pay is available for those with Certified Home Health Agency ( CHHA ) experience. This a full-time direct hire opportunity offering competitive salary, paid medical insurance , and PTO .

Key Responsibilities:
Provide skilled nursing care in patients' homes, ensuring individualized and patient-centered treatment.
Conduct assessments, develop care plans, and monitor patient progress.
Collaborate with interdisciplinary teams to coordinate and optimize patient care.
Educate patients and families on managing chronic conditions, medication adherence, and home safety.
Maintain accurate documentation and ensure compliance with home health regulations.

Requirements:
Active Registered Nurse (RN) license in NY.
Experience in home health or a Certified Home Health Agency (CHHA) preferred.
Strong clinical assessment and critical thinking skills.
Excellent communication and patient education abilities.

Please note- The annual base salary range is $100k to $110k. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
