Erie Family Health Centers, a top workplace in Chicago and suburbs, is looking for a valuable addition to our
Patient Program and Support Services. The Community Health Worker (CHW) - Health Promotions supports the Health Promotion programs with innovative methods to increase patient and community participation in health promotion pool, and through MyChart, texts, mailings, and telephone calls. This position supports outreach, engagement, and recruitment.

Compensation is based on each candidate's experience, skills and education within the range identified for the role. Candidates who meet the minimum requirements of the role will start at entry in the range. Any additional skills, experience and education will be reflected in the compensation offered.
Main Duties & Responsibilities - Manages all incoming in-basket messages to health promotion pool and incoming telephonic inquiries from patients. Responsible for the distribution, program inventory, and administration of Reach Out and Read program.
- In collaboration with Senior Coordinator, Health Promotion, obtains BI reports necessary to complete National Reach Out and Read bi-annual progress report.
- In Collaboration with Senior Coordinator and marketing team, organizes health educational material, program flyers, for staff and patient access in different platforms.
- Responsible for executing program initiatives, health promotion outreach via telephone, MyChart, or text messages, and promoting health promotion programs services/groups via Erie Newsletters, emails, and/or in-person interactions across Erie sites.
- Provides resources to patients based on identified needs or links them to other teams that may assist.
- Assess patient's readiness and educational needs on an individual and/or group basis to link patients with most aligned support and to support recruitment for group programming and program goals/ grant objectives
- Participates in approved community outreach and education events including health fairs and community workshop presentations.
- Conducts all assigned health education groups monthly with patients interested in lifestyle modification strategies.
QualificationsEducation - High School diploma or equivalent required
- Associates degree, some college or bachelor's degree a plus
Skills and Knowledge - Bilingual in fluency (English/Spanish) required.
- Two (2) plus years of Health Promotions experience preferred
- One (1) year of outreach, education, community health, or customer service experience
- Strong computer skills required; expert in Microsoft Office, excel and ability to learn other electronic databases (Epic electronic medical record system, etc.)
- Attentive to detail, problem solving, innovative, and critical thinking skills.
- Previous experience working in a healthcare setting preferred
- Experience conducting patient assessments preferred.
- Phone based outreach experience preferred.
- Experience identifying, referring to, and working with community-based organizations preferred.
