Job Summary
  • The Primary Care Health Home Health Navigator assists in managing PCHH patients by demonstrating a patient-centered approach for coordinating the healthcare needs and wellness goals of the PCHH program patients. The Health Navigator works closely with healthcare providers, patients, and the PCHH team to develop and implement plans to address medical, behavioral, and social needs. The Health Navigator manages care transitions, eliminating barriers to care, and fostering effective communication among care team members. In addition to managing the clerical needs of the program including processing enrollment and discharge paperwork to the state, this position manages multiple program spreadsheets and processes reports received by the state.

Job Requirements
  • Education
    • Required: High School Diploma or Equivalent
  • Experience
    • No previous experience required
    • Preferred: Previous experience with Electronic Health Records and /or 2-3 years experience working in a Family Practice out-patient setting
  • Skills
    • Strong organizational and multitasking abilities
    • Excellent telephone and interpersonal communication skills
    • Ability to multi-task and prioritize; strong organizational skills
    • Strong knowledge of medical terminology
    • Computer experience required
    • Evaluation of patient, Care plan oversight and Care Coordination required.
