Overview
Summary: Under the supervision of a licensed RN, the graduate nurse is responsible for utilizing the nursing process to provide direct patient care to an assigned group of patients during a shift and periodizing the delivery of care using time and resources efficiently integrating an evidence and standards based framework model.

Other information:
- Associates in Nursing or Bachelors of Science in Nursing
- Graduate of an academic School of Nursing required
- Minimum ADN or BSN Preparation
- State permit to practice as a Graduate Nurse required
- Degree from an accredited academic school of nursing preferred
- Permit to practice at the specific PHS facility is completed and approved by the NMBON
- Permits to practice are valid until the NCLEX examination are disseminated but not to exceed the expiration date on the permit
- Under the direct supervision of a licensed RN who is physically in the same facility and/or on the same unit who can observe, direct, and evaluate the performance of the GN
- The Graduate Nurse must be scheduled to take the NCLEX exam
- Graduate RNs educated in the United States may take the exam a maximum of three times within three years of first being eligible for to sit for the exam
- The applicant must wait 45 days to retest after failing the exam and meet all the NCLEX requirements for retaking the exam (Nurse Practice Act, NM Code R. 16.12.2.10 NMAC)

Responsibilities
Responsibilities:
- Graduate Nurse, Under the direct supervision of a Registered Nurse, assumes these responsibilities
- Per unit and practice standards, provides direct patient care by assessing, planning, implementing, evaluating, and documenting individual patient needs, actions taken, and patient responses.
- Coordinates patient care through collaboration with patient/family and healthcare team
- Performs assessment per evidence-based standards and documents in EMR and any designed documentation platforms
- Initiates, utilizes and documents appropriate protocols and standards of care
- Administers medication and treatment as prescribed by physician, provider, or standard protocols and documents according to policy
- Identifies and meets patient and family learning and discharge needs on admission and throughout episode of care
- Evaluates patient progression towards outcomes per unit standards and documents
- Performs at basic competency levels in emergency situations
- Delegates or assigns aspects of patient care to healthcare team members commensurate with their validated competency to perform task
- Assumes responsibility for clinical supervision of LPNs and non-licensed assistive personnel
- Provides leadership through communication about and validation of patient care
- Assists with various procedures, treatment, surgical techniques or assigns to other healthcare team members as appropriate
- Completes yearly education per PARTE and PACCER modules and any unit specific requirements on time
- Demonstrates competencies in required psychomotor skills
- Assumes professional responsibility for current nursing structure and clinical standards of care
- Incorporates PHS MVV and CARES in caregiving
- Supports DON philosophies of shared governance and models of care deliveries
- Mandatory yearly in-service to include BLS, safety training, bloodborne pathogen/infection/TB training
- Participates in unit/department governance
- Documents own continuing education
- Participates in quality improvement activities
- Completes mandatory in-services, competencies and Graduate Nurse requirements on time
