Primary Care PACE Physician Assistant
Join our robust team as a primary care provider for a San Diego based Program of All-Inclusive Care for the Elderly (PACE), providing high quality, compassionate geriatric care in an outpatient setting. Enjoy lower volume and more time with your own panel of patients.

Description
-Provide acute, chronic, preventive, curative and rehabilitative medical care to patients -Manage transitions of care, chronic disease, advanced goals of care discussion and palliative management -Refer participants to other in-house providers, including behavioral health -Document visits in the electronic medical record
Hours -Monday through Friday -Full time
Location Vista, California (north county San Diego)
Requirements
• 2 years post completion of clinical training
• Board certification
Preferred
• Employment experience in a community health center
• Experience working with elderly population
• Leadership experience a plus.
