Join Our Team as a Nurse Practitioner in Geriatric Medicine
We are excited to invite a dedicated Nurse Practitioner with a passion for geriatric care to our innovative facility in Philadelphia, PA. Our organization is part of the PACE (Programs of All-Inclusive Care for the Elderly) model, which beautifully integrates healthcare services to assist older adults in living independently within their communities. This role offers a unique opportunity to work within a compassionate team that is transforming healthcare for the elderly. The facility is designed to meet State and Federal safety requirements and includes adult day programs, medical clinics, and therapy facilities.
Job Responsibilities:
- Provide primary medical management in collaboration with the PACE Organization's Physician(s).
- Conduct assessments in collaboration with the physician as needed.
- Develop and implement appropriate plans of care for participants.
- Evaluate participant physical complaints and provide appropriate treatment.
- Offer caregiver teaching and education.
- Function as a member of the Interdisciplinary Team (IDT).
- Oversee daily operations of the PACE Organization's Clinic.
- Perform rounds in contracted nursing homes.
- Maintain communication with IDT regarding participant medical conditions.
- Demonstrate superior communication and organizational skills.
- Exhibit adaptability and problem-solving abilities.
Qualifications:
- ANCC/AANP Board Certified.
- Active Pennsylvania license.
- 5-7 years of NP experience, preferably in long-term care.
- Minimum one year working with frail or elderly populations.
- BLS and DEA Registration are required.
- Certification as a Nurse Practitioner is mandatory.
- High accountability with decision-making abilities.
- Excellent written and verbal communication skills.
- Comprehensive proficiency in Microsoft Office and electronic communication tools.
License and Certification Requirements:
- ANCC/AANP Board Certification - REQUIRED
- Active PA Nursing License - REQUIRED
- BLS Certification - REQUIRED
- DEA Registration - REQUIRED
- Certification as a Nurse Practitioner - REQUIRED
- Preferable: Clean/Clear malpractice history.
