This full-time positionoffers a fulfilling opportunity to provide compassionate, high-quality care togeriatric patients in a supportive and collaborative healthcare environment.The schedule includes working Monday through Friday, 8:00 AM to 4:00 PM, withrotating on-call responsibilities.
Responsibilities:
In this role, you will provide comprehensive medical care to geriatricpatients, including the assessment, diagnosis, and treatment of acute andchronic conditions. You will collaborate with an interdisciplinary team todevelop and implement individualized care plans and address patient and familyconcerns, offering education on managing health conditions and optimizingquality of life. On-call duties include one week on, one week off, with 24-hourcall coverage on weekends. Additionally, you will maintain accurate anddetailed patient records in compliance with state and federal regulations.
Schedule: - Full-time: Monday-Friday, 8:00 AM to 4:00 PM
- Rotating call schedule: One week on, one week off
- Weekend call coverage: 24 hours
Requirements: - Active Nurse Practitioner certification through AANP or ANCC.
- Valid Indiana state license to practice as a Nurse Practitioner.
- Experience in geriatric medicine is highly preferred.
