Deaconess is seeking a full-time ACNP, FNP, or PA to join the Geriatric Workforce Enhancement Program. This is a fully grant funded position to serve as the Clinical Program and Evaluation Specialist for our Geriatric Workforce Enhancement Program. The GWEP is a partnership between USI, Deaconess, and AAA sites to improve the care for older adults in our community. Deaconess role is largely focused on geriatric care in the primary care setting. The current 5-year grant program is geared towards clinical practice, social determinants of health and health inequities and geriatric education and training needs of health workers which will allow us to increase the knowledge of PCP providers and embed the 4 Ms workflows into their practice to strengthen the care for our geriatric population. Typical working hours will be from 8:00 am - 4:30 pm. Candidates should be licensed in the State of Indiana and certified by the ANCC/AANP as a Nurse Practitioner or certified by the NCCPA as a Physician Assistant.
This position will focus on supporting our 5 identified rural and underserved clinics in the first year and will expand to additional sites in years 2-5. The provider may help with acute, AWV and TCM visits in the respective clinics for .4 of their FTE.
Other duties of NP to support the GWEP efforts:
- Serves as liaison to clinical teams and primary care sites, supporting the development of age- and dementia-friendly clinics.
- Manages clinical data collection, improvement, and reporting for HRSA requirements, internal projects, and quality indicators.
- Collaborates with USI GWEP Data and Evaluation Manager and keeps GWEP steering committee informed of clinic progress.
- Serves on GWEP steering committee and co-leads the Interprofessional Clinical Training Team.
- Works closely with GWEP CC and Institute of Healthcare Improvement.
- Supervises the Deaconess CHW and Deaconess CMA.
- Will practice as a NP provider in virtual/in-person GWEP Training and TTOUR clinic sites at a .2 to .4 FTE effort.
- Quality Assessment and appropriate workflow and training modifications will occur as a result of the NP practice.
