Dates needed: Dec. 24, 26-27
Hours: Tue-Thurs 7a-5p, Fri 7a-12p
Clinical Staff : 1MCD/1 Associate, 1 Hyg, 3 DAs
Require Procedures : Surgical Extractions preferred
Avg PPD: 20-25
