What you will do:
The RN (Registered Nurse) Case Manager provides case management and nursing services to Hudson Valley Hospice patients and their families and to ensure that those services are consistent with the philosophy, goals, objectives and policies of the Agency. As an RN Case Manager, you are assigned to a specific team to work within. Each Registered Nurse (RN) Case Manager provides pain and symptom management, recommendations and collaborates with the other Interdisciplinary Team members.
Perform initial and ongoing patient/family, physical, emotional, psycho-social, cultural and spiritual assessments to ensure and coordinate the delivery of comprehensive, holistic services by the Interdisciplinary Hospice Team.
Provide palliative nursing care and symptom control in accordance with physician orders and IDG plan of care.
Documents and enters into EMR all interventions of phone and home visits within 24 hours of the intervention. Medication changes will be entered by the end of the shift.
Provide anticipatory guidance to the patient/family to assist in preparing for coping with events as they occur.
Teach, supervise and counsel patients/families/care givers regarding physical care, observing return demonstration of procedures as appropriate.
Communicates patient status to family members and or caregivers.

Education & Experience we are looking for:
Graduate of an accredited nursing school.
Valid New York State nursing license.
Two years nursing experience required
One year medical-surgery experience highly preferred
Work Environment:
Work in various home environments, nursing homes or hospitals that Hudson Valley Hospice is contracted with
Moderate to High level of possible exposure to infectious agents and possible unsafe environments
Moderate amount of travel by car
Routine duties involve potential for direct exposure to blood or body fluids.
Valid New York State drivers license with reliable transportation.
Hours: Monday-Friday (8:30am-4:30pm); on call rotation obligation(3-4 times per years) ; FLEXIBILITY within the schedule of the position
