We are seeking a LocumsNurse Practitioner or Physician Assistant to join our geriatric care team in Manhattan, NY. This is a flexible, part-time or full-time role providingin-home primary care to PACE (Program of All-Inclusive Care for the Elderly)participants. The ideal candidate is experienced in home-based or geriatriccare, patient-focused, and comfortable working independently in the field.
Position Highlights:
- Flexible Schedule: 2-4 daysper week, Monday-Friday
- No nights or weekends
- Low patient volume: 3-5patients per day
- Hourly Compensation: $75-$80per hour

Responsibilities:
- Provide comprehensiveprimary care services in participants' homes
- Conduct routine and urgentvisits, including history, physical exams, medication reconciliation, and careplanning
- Collaborate withinterdisciplinary PACE care team to ensure coordinated, high-quality care
- Document clinical encountersin a timely and accurate manner
- Support chronic diseasemanagement, preventative care, and advance care planning
Requirements:
- Active Nurse Practitioner orPhysician Assistant Board Certification
- Medicare & MedicaidProvider ID - Required
- Minimum 2 years of clinicalexperience preferred, ideally in geriatrics, primary care, or home caresettings
- Spanish-speaking candidatesstrongly preferred, but not required
- Strong interpersonal andcommunication skills
- Comfortable practicingindependently in home-based environments
