Family Practice/Primary Care Physician Assistant

-Hybrid, full time schedule in patient homes and at home: Monday through Friday 8:30 - 5:00 pm
-Home care or hospice experience preferred
-Chronic disease management experience preferred
-Must be willing to travel up to 60 miles from home
-Must be open to traveling to Utah and occasionally New Mexico every 6-8 weeks for 2-3 day periods
