Family Practice/Primary Care Physician Assistant

We are working on an exciting new opportunity with a growing healthcare company that is seeking travel Nurse Practitioners or Physician Assistants in Pennsylvania/Delaware/New Jersey to work visiting patients at home. -Full time schedule, Monday-Friday (home on weekends)
-2-3 weeks per month doing in home wellness and initial visits
-Home care or hospice experience preferred
-Chronic disease management experience preferred
-Travel experience is preferred, must be flexible to travel needs
-MUST be licensed in Pennsylvania
-Can live in DE, NJ, or PA
