NP/PA Family Medicine opening in New HampshireFull TimePrimary Care ProviderColebrook, NhDuties:Provide comprehensive medical care including physical exams, diagnoses, and treatment plans; develop personalized health plans for patientsManage long-term treatment plans for patients with chronic illness, ensuring regular follow-ups and monitoring; manage acute care and chronic medical conditions and commodities.Offer preventative care services such as immunizations, screenings (e.g., cancer, cholesterol, blood pressure), and well-ness check-ups.
