APP Family Medicine Opening in Colebrook, NH-Full Time & Permanent Position.-Experience required.Job Description-Provider must have minimum of 2 years in practice in outpatient primary care, rural health clinic experience preferred.-Able to work autonomously.-Must be Board Certified.-Office location is in Colebrook NH.-Direct patient care time will be split between acute care and established patient panel.-Replacement for provider that is retiring.Primary Care:-Pediatric through Geriatric.-High elderly population.-Experienced in Chronic Disease Management.-EMR is Meditech Expanse, Dragon dictation.-Supported by an RN Team leader and medical assistant.
