Kaizen Clinical Partners in the Rio Grande Valley are currently seeking a Nurse Practitioner to join their expanding primary care/home health group. Practicing and recent resident graduates to be considered; experience in outpatient/primary care preferred.
Comprised of several primary care office locations across the Rio Grande Valley, Kaizen Clinical Partners provides a strong culture of value-based care to their patients and families.
Job Details:
- Set your own schedule
- Provide comprehensive primary care to patients in their homes
- Manage chronic illnesses in a complex elderly population
- Facilitate referrals and coordinate care with other agencies; If needed, adjust services, such as home care
- Develop and implement patient management plans
- Dedicated Medical Assistant to manage administrative duties
- Spanish speaking preferred
