Nurse Practitioner - Annual Health Assessment Medford, Oregon
Full-Time On-Site & In-Home Visits Mon-Fri, 8am-5pm PST
$15K Sign-On Bonus or $15K Relocation Assistance RecruitWell is seeking a compassionate and experienced Nurse Practitioner (NP) to join a mission-driven team delivering holistic, value-based care to Medicare patients in the Medford, OR area. This permanent, full-time role focuses on annual wellness assessments in both outpatient clinic and in-home settings. Position Highlights
•   Conduct annual Medicare wellness visits in clinic or at patient homes (within 30-mile radius)
•   Manage high-risk and post-acute patients to reduce avoidable hospitalizations
•   Perform HCC coding , screenings, discharge planning, and medication reconciliation
•   Collaborate with an interdisciplinary team (social workers, care managers, MD advisors)
•   Average 10-12 clinic or 5-6 home visits per day
•   Complete EMR documentation within 72 hours Qualifications
•   Active Oregon NP license
•   Certified by ANCC or AANP
•   2+ years NP experience in Primary Care, Family Medicine, Internal Medicine, Geriatrics, or Palliative Care
•   DEA license , CPR/BLS certification, and valid Oregon driver's license
•   Experience with HCC coding , managed care, or home health is a plus Ideal Candidate Traits
