We are seeking a skilled Family Practice Nurse Practitioner for a 60-day locum tenens assignment at a Multi-Specialty Clinic in West Virginia, starting 06/02/2025. The ideal candidate will work Monday through Friday, 8 AM to 5 PM. This role involves providing care in a long-term nursing home facility, managing acute and chronic conditions, developing treatment plans, and monitoring for early changes in patients' conditions. Responsibilities include conducting scheduled follow-up visits, monthly checkups, acute care, admissions, and discharges for a diverse patient population, including some psychiatric and substance abuse patients. The position requires proficiency in GEHRIMED software, an active West Virginia nursing license, and the ability to achieve 25 RVUs per day.

Responsibilities and Duties:

  • Provide acute and chronic care management in a long-term nursing home setting
  • Develop and implement treatment plans for patients
  • Monitor and identify early changes in patients' conditions
  • Conduct scheduled follow-up visits and monthly checkups
  • Manage acute care cases as needed
  • Perform patient admissions and discharges
  • Utilize GEHRIMED software for patient management and record-keeping
  • Maintain accurate and detailed patient records
  • Collaborate with facility staff to ensure efficient operations
  • Provide care for psychiatric and substance abuse patients as required
  • Achieve daily target of 25 RVUs (Relative Value Units)
