Family Practice/Primary Care Nurse Practitioner

What you will do:

• Provide primary medical coverage in collaboration with physicians as needed.
• Perform initial, semi-annual, and annual assessments of participants, implementing appropriate plans of care.
• Participate in discharge planning for acute and long-term placement.
• Evaluate and treat participants during acute illness, prescribe medications, therapies, and other treatments.
• Refer participants to medical specialists as indicated.
• Conduct nursing home or home visits as needed.
• Collaborate with medical director and clinic supervisor to formulate clinical policies, procedures, and standards of care.
• Assist with the development of standards of care, monitor and evaluate patient care practice and service delivery, and provide guidance and training to staff regarding medical and quality assurance issues.
• Advise the medical director and/or primary care provider to establish better accountability of services to participants and referral sources.
• Act as a resource during the intake of new participants and in the day-to-day operation of the center programs.

Submission Requirements:

• Master's degree in Nursing
• Minimum of five years of experience, preferably in long-term care
• Active PA License
• BLS, CDS, DEA certifications
• Board Certified by ANCC or AANP
• Certification as a Nurse Practitioner with prescriptive authority
• A minimum of one year working with the frail or elderly population strongly preferred
• Must have a valid driver's license, proof of insurance, and have means of transportation

Certifications:

• AANP, ANCC, BLS, Controlled Substance (CSR/CDS), DEA
