You'll work alongside a collaborative team-including a dedicated WholeHealth Coach-in a setting designed to support quality over quantity.
Position Highlights: - Compensation:$125,000-$135,000 annual salary
- Patient Panel: 10-15patients per day - time to focus on what matters
- Schedule: Monday-Friday,with occasional regional after-hours coverage rotation
- Model of Care: Medical Homemodel with onsite lab and pharmacy for convenience
What You'll Do: - Deliver compassionate,patient-centered care for chronic and acute conditions
- Collaborate with patientsand an integrated care team to create personalized care plans
- Conduct comprehensivephysical exams, order and interpret diagnostic tests, and prescribe treatments
- Promote population healthinitiatives like annual wellness exams and health coaching
- Lead and inspire team-basedcare through communication, process improvement, and clinical insight

What We're Looking For: - Board-certified NP with abackground in family medicine, internal medicine, or primary care
- Active and unrestrictedTexas license and DEA
- 3-5 years of primary careexperience
- 2+ years in case managementor population health preferred
- Familiarity with EMR systems(NextGen preferred) and Medicare Advantage/HCC documentation
