We are seeking a skilled Family Practice Nurse Practitioner for a locum tenens position with a Home Health agency in Georgia. This 60-day assignment starts on 06/02/2025, with a flexible schedule ranging from 8:00 AM to 8:00 PM, with a minimum of 8:00 AM to 5:00 PM shifts available. The ideal candidate will be comfortable conducting Medicare Assessments, including reviewing medical history and medications, completing HRA/Charts, and performing required screenings such as Dexa, Retinal Screening, and FOTB. This role offers an opportunity to provide in-home patient assessments and screenings, ensuring comprehensive care for home health patients. The Nurse Practitioner will be responsible for conducting thorough assessments, documenting findings, and contributing to patient care plans.

Job Responsibilities and Duties:


• Conduct comprehensive Medicare Assessments in patients' homes


• Review and document medical history and current medications


• Complete Health Risk Assessment (HRA) charts accurately


• Perform required screenings, including Dexa, Retinal Screening, and FOTB


• Receive training on specific screening procedures prior to conducting them


• Utilize Focus Care EMR system for patient documentation


• Collaborate with the home health team to ensure quality patient care


• Provide patient education on health maintenance and disease prevention


• Adhere to all agency policies and procedures for in-home assessments


• Maintain accurate and timely medical records


• Participate in any required training or orientation sessions


• Communicate effectively with patients, families, and healthcare team members


• Contribute to the development and implementation of patient care plans


• Stay updated on best practices in home health assessments and screenings


Additional Information:


• Must be licensed as a Nurse Practitioner in Georgia


• Family Practice certification required


• Experience with home health or in-home assessments preferred


• Ability to work independently and manage time effectively


• Strong clinical assessment and documentation skills


• Excellent communication and interpersonal skills


• Comfort with using electronic medical record systems, specifically Focus Care EMR


• Flexibility to accommodate varying schedules and patient needs


• Possibility of assignment extension beyond the initial 60 days
