Key Responsibilities
- Provide high-quality, patient-centered primary care to patients of all ages, including preventive care, chronic disease management, and acute care.
- Conduct patient assessments, order diagnostic tests, prescribe medications, and develop individualized treatment plans.
- Collaborate with a multidisciplinary team of healthcare professionals, including nurses, behavioral health specialists, and case managers.
- Maintain accurate and timely documentation in the electronic medical record (EMR) system.
- Participate in quality improvement initiatives and contribute to organizational goals.
- Educate patients and families about health maintenance, disease prevention, and treatment options.
- Adhere to FQHC policies, procedures, and guidelines to ensure compliance with federal and state regulations.
Qualifications
- Board-certified or board-eligible in Family Medicine.
- Licensed to practice medicine in Indiana or eligibility for licensure.
- DEA certification.
- Commitment to serving underserved populations with cultural sensitivity and compassion.
- Strong communication and interpersonal skills.
- Bilingual proficiency is a plus but not required.
