Position Summary:

We are seeking a dedicated Nurse Practitioner to deliver preventive and primary care services in patient homes. This role emphasizes providing care in environments where patients are most comfortable, aiming to improve health outcomes and reduce unnecessary hospital visits.

Key Responsibilities:

  • Provide comprehensive medical care and care coordination to diverse patient populations, including adults, geriatrics, women's health, and pediatrics.
  • Conduct thorough evaluations, including history and physical exams, to identify gaps in care and address preventive needs.
  • Diagnose and manage chronic and acute conditions, utilizing sound clinical judgment.
  • Order and interpret diagnostic tests and specialty referrals as necessary.
  • Develop, document, and implement individualized care plans using an electronic medical record system.
  • Prescribe medications and perform procedures within the scope of practice.
  • Conduct in-person visits across various settings, adapting to patient needs and business requirements.
  • Participate in community outreach initiatives, such as "pop-up" clinics, to build relationships and address healthcare disparities.
  • Engage in post-discharge follow-up care to minimize hospital readmissions and emergency room utilization.
  • Collaborate with interdisciplinary teams to enhance care delivery and patient outcomes.
  • Participate in on-call rotations for urgent lab results and pharmacy inquiries.

Qualifications:

Education: Master's degree from an accredited Family Nurse Practitioner program.

Licensure and Certifications:

  • Active, unrestricted Family Nurse Practitioner license in the state of practice.
  • National certification through AANP or ANCC.
  • Basic Life Support (BLS) certification for Healthcare Providers.
  • Valid and unrestricted driver's license.

Experience:

  • New graduate NPs are encouraged to apply.
  • Preferred: 3-5 years of experience in home health, community health, or public health settings.

Skills:

  • Proficiency with electronic medical records and Microsoft Office applications.
  • Strong clinical reasoning and independent decision-making abilities.
  • Excellent communication and interpersonal skills.
  • Ability to lift up to 50 lbs. and navigate multi-level buildings.
  • Willingness to obtain additional state licenses as needed.
