Contracted ECM Case Manager Location: Palo Alto, CA
Schedule: Monday - Friday, 8:30 AM to 5:00 PM
Pay: $24 to $33/hour (dependent on incoming experience
Contract timeline: Estimated 1-3 months
Key Responsibilities: - Provide comprehensive case management services to a designated caseload of clients under the ECM model
- Conduct needs assessments and develop individualized care plans
- Coordinate and monitor care across primary care, specialty providers, behavioral health, and social support services
- Engage clients through in-person visits, phone calls, and field-based outreach
- Assist with transitions of care, including discharges from hospitals or treatment centers
- Document all services and communications in accordance with program and HIPAA requirements
- Collaborate with interdisciplinary care teams, community providers, and family members
- Advocate for client needs and connect them with appropriate community resources
Requirements: - Bachelor's Degree (required; preferably in Social Work, Psychology, Public Health, or a related field)
- Bilingual in Spanish and English (required)
- Minimum of 3 years of relevant experience in case management, care coordination, or working with vulnerable populations
- Strong knowledge of Medi-Cal systems, local resources, and social determinants of health
- Excellent communication, documentation, and organizational skills
- Ability to work independently and as part of a collaborative team
- Reliable transportation and willingness to conduct field work as needed
