Discharge Planner

The LPN Case Manager assists RN staff in developing discharge plans and completing clinical assessments for patients needing post-acute Medicaid services in states like Virginia, West Virginia, and Maryland.

•  Responsibilities include pre-screens, admissions, insurance authorizations, and Level 2 assessments.

•  Coordinates with community resources to ensure seamless patient transitions across care levels.

•  Requires graduate nursing education, 2 years bedside experience, WV LPN licensure, and BLS certification.
