Job Summary
The Hospice Social Work Director is responsible for overseeing all hospice social work services, ensuring compliance with federal, state, and accreditation standards. This role requires a compassionate leader with hospice experience, excellent clinical judgment, and the ability to mentor and develop a high-performing team of social workers.
Key Responsibilities Leadership & Supervision:
- Lead, supervise, and support the hospice social work team, providing ongoing training and mentorship.
- Develop and implement policies and procedures in compliance with CMS Hospice Conditions of Participation (42 CFR Part 418) and Oregon Administrative Rules (OAR to ).
- Participate in Interdisciplinary Team (IDT) meetings to ensure a holistic approach to patient care.
- Conduct performance evaluations and provide professional development opportunities for social work staff.
Patient & Family Support:
- Provide psychosocial assessments, emotional support, and crisis intervention for hospice patients and their families.
- Facilitate advance care planning, end-of-life decision-making, and bereavement support.
- Address complex family dynamics, grief, and loss concerns with compassion and professionalism.
- Coordinate with chaplains, counselors, and community resources to enhance patient support.
Regulatory Compliance & Documentation:
- Ensure all patient records, assessments, and interventions are documented accurately and in a timely manner per Medicare, Medicaid, and hospice regulatory requirements.
- Develop and implement Quality Assurance & Performance Improvement (QAPI) initiatives to enhance social work services.
- Educate patients and families on hospice benefits, rights, and available community resources.
Community Outreach & Partnerships:
- Establish relationships with healthcare facilities, social service agencies, and mental health organizations to enhance patient care.
- Represent Everlight Hospice at networking events, educational programs, and public forums.
- Advocate for hospice care awareness and patient rights in the community.
Qualifications & Requirements:
- Master's Degree in Social Work (MSW) required; Licensed Clinical Social Worker (LCSW) preferred.
- Minimum 1 year of hospice or palliative care experience (leadership experience preferred).
- Strong understanding of hospice regulations, Medicare/Medicaid guidelines, and end-of-life care principles.
- Excellent leadership, organizational, and communication skills.
- Ability to work collaboratively in an interdisciplinary team setting.
- Proficiency in electronic medical records (EMR) documentation.
- Valid driver's license and reliable transportation for home visits as needed.
