Position Overview:

We are seeking an accomplished Home Care-Home Health Administrator to lead the launch and management of our new Home Care and Home Health Agency based in Houston. Key Responsibilities:
  • Oversee the day-to-day clinical operations for in-home caregiver services
  • Direct the strategic planning, operations, and evaluation of agency performance
  • Ensure regulatory compliance and adherence to Medicare Conditions of Participation (CoPs)
  • Help ensure the company is prepared for State and Accreditation Surveys
  • Recruit, train, mentor, and retain high-performing clinical and administrative staff
  • Drive continuous improvement initiatives (QAPI) to enhance quality care outcomes
  • Oversee business development efforts, fostering robust relationships with community partners, referral sources, and stakeholders
  • Manage operational budgets, financial outcomes, and report regularly on business-critical metrics
  • Implement advanced technological solutions and innovations to improve patient care and operational efficiency
  • Maintain exceptional client satisfaction and caregiver engagement levels
  • Proactively address operational risks, ensuring prompt, comprehensive resolution
  • Ensure all care plans are personalized, compliant with state regulations, and updated regularly
  • Supervise and provide guidance to RNs, LVNs, and caregiving staff
  • Conduct initial assessments and ongoing evaluations of client needs
  • Ensure accurate documentation and timely reporting in accordance with company policies and Texas Department of Aging and Disability Services (DADS) guidelines
  • Lead training and development initiatives for caregivers on best practices, safety, and patient care
  • Participate in hiring, onboarding, and performance evaluations of clinical staff
  • Monitor infection control, incident reporting, and overall risk management
  • Act as liaison between families, physicians, and care teams to ensure coordination and quality of care
  • Stay updated on industry regulations and ensure ongoing compliance
Essential Qualifications:
  • Minimum 3-5 years of nursing experience, with at least 2 years in a leadership or supervisory role
  • Current, unrestricted RN license in the state of Texas required
  • Bachelor of Science in Nursing (BSN) required; Master's degree preferred
  • Strong knowledge of federal and state of Texas Home Care/Health regulations, Medicare CoPs, and reimbursement practices
  • Experience in home health, hospice, or in-home caregiving strongly preferred
  • Exceptional leadership capabilities, promoting staff engagement, professional development, and operational excellence
  • Excellent communication and interpersonal skills, adept at engaging with diverse stakeholders
  • Proficient with electronic medical records (EMRs) and documentation practices
  • Ability to multitask, prioritize, and solve problems in a fast-paced environment
  • Proven ability to implement and manage innovative technologies in healthcare settings
