Summary
The Servant Center's Disability Assistance Program (DAP) assists un- and underinsured clients to file for SSI/SSDI benefits. SOAR (SSI/SSDI, Outreach, Access and Recovery) is an evidence-based model in which we help those experiencing homelessness to file for SSI/SSDI benefits. We also assist with Medicaid and Food Stamp applications as needed for SOAR and DAP clients. The department submits 200+ SSI/SSDI applications each year.
Job Purpose
- The Director of Disability Services oversees the day-to-day operation of the Disability Assistance Program, including staff supervision, monthly reports, attending relevant meetings, etc.
- The Director will also maintain a small SOAR case load. The SOAR (SSDI/SSI, Outreach, Access and Recovery) Case Worker provides outreach and support to homeless adults with mental illness for the purpose of expediting the SSI/SSDI and medical benefits application process.
Essential Functions
Disability Assistance:
- Strengthens relationships with partner agencies.
- Direct and supervise all DAP staff, including Disability Specialists, and the administrative assistant.
- Maintains a database to track the progress of claims and their status.
- Submit reports as required by grantors and contractors.
- Provides claim assistance to clients.
SOAR:
- Supervise the full-time SOAR Disability Specialist.
- Provides outreach to clients in various community locations.
- Collects medical and other evidence for inclusion in SSI/SSDI applications.
- Completes assessments and writes medical summary reports as needed.
- Complete SSDI/SSI applications using established policies and procedures of the SOAR program.
- Deliver or electronically transfer all needed applications and back-up documentation to SSA.
- Maintain regular communication with clients to ensure follow-through with SSI/SSDI applications.
- Collect, record, and report data on SOAR outcomes.

Qualifications
- Bachelor's Degree required.
- Understanding of the Social Security Disability process is a plus.
- Computer literacy, with proficiency in MS Office, Excel, internet.
- Ability to communicate effectively with people from a wide variety of socio-economic circumstances in a courteous and professional manner.
- 3-5 years of experience in a non-profit agency required.
- SOAR Certification: or willingness to obtain SOAR certification.
- Experience working with the homeless and/or mental health population.
- Reliable transportation and a valid, unrestricted driver's license.
- Demonstrated ability to work independently and in cooperation with others.
- Ability to organize, prioritize tasks quickly, and multitask.
- Strong interpersonal skills in working with diverse cultural and ethnic populations.
- Ability to set and adhere to boundaries.
Working Conditions
Generally, works in an office environment but may occasionally be required to perform job duties outside of the typical office setting, such as the hospital, shelter, outreach location, or a client's home.
Physical Requirements
- This is not a physically demanding job.
- The person in this position needs to occasionally move about inside the office to access file cabinets, office machinery, etc.
- The person will constantly operate a computer and other office productivity machinery, such as a calculator, copy machine, computer, and printer.
- The person in this position would rarely lift 10 or more.
