In this leadership role, you will oversee the coordination of patient care from admission to discharge, ensuring that all patients receive efficient, effective, and high-quality care. You will collaborate with physicians, nurses, and social workers to optimize patient outcomes, reduce length of stay, and manage hospital resources effectively.

• Lead and manage the case management department, including utilization review, discharge planning, and care coordination.
• Develop and implement strategies to improve patient flow and reduce hospital readmissions.
• Collaborate with the interdisciplinary team to ensure continuity of care across all phases of treatment.
• Ensure compliance with regulatory requirements, accreditation standards, and hospital policies.
• Monitor departmental performance, set quality and financial goals, and report on key performance indicators.
• Recruit, mentor, and develop case management staff to support the delivery of high-quality care.
• Engage with patients and families to address concerns and provide education about care transitions.

Candidate requirements

• Current RN license in Colorado.
• 5+ years of nursing experience, with at least 2 years in a case management or leadership role.
• Knowledge of case management, utilization review, and discharge planning processes.
• Strong leadership, communication, and organizational skills.
• Bachelor's degree in nursing (BSN) is required; MSN or related advanced degree preferred.
• Case Management Certification (CCM) or ACM certification is preferred.
