Director of Care Management

The job entails leading the Care Management and Social Work Department to ensure quality patient care, adherence to standards, and balancing clinical and financial goals.
•  Responsibilities include managing care teams, developing care programs, analyzing data, advocating for patients, and aligning strategic objectives.
•  Requires a BSN or ADN, 10+ years of RN clinical experience with 3+ in case management, and RN licensure in NM. CCM certification is preferred.
