A Brief OverviewThe Director of Care Coordination at Children's Nebraska is responsible for the development, implementation and oversight of strategies, programs and services focused on providing optimal patient/family centered integrated experience. Ensures Family Services programs align with strategic and operational goals and objectives. Working with leaders at the operating unit level and system level to review and enhance Family Services/Service Excellence, ensuring high-quality, patient-centered care, and continuity of care across all pediatric services. Is responsible for leading transformational change at all levels of the operating unit, fostering a collaborative environment to effectively identify, prioritize and implement operating unit opportunities for best practices related Family Services/Patient Experience in alignment with system priorities and goals across all pediatric services. Operating units of responsibility includes Social Work, Case Management, Utilization Management, Spiritual Care, Child Life & Healing Arts, and Project Austin.
Essential FunctionsLeadership and Strategic Planning:
- Responsible and accountable for the strategic planning, coordination and evaluation of Care Coordination services provided to patients and families, ensuring objectives are in alignment with Children's Nebraska's mission and values.
- Leads, mentors, and supervises department leadership and the multidisciplinary team members in their specialty roles of case managers, social workers, spiritual care, child life, healing arts, utilization management, and Project Austin.
- Foster a culture of teamwork, accountability, and continuous improvement within the department.
Care Coordination Program Management:
- Oversees the development, implementation and collaboration throughout the organization to advance and embed family centered integrated coordination of complex pediatric patients care across inpatient, outpatient, and community settings.
- Ensure a seamless transition of care from acute care settings to home or community-based services, working closely with clinical teams and external partners.
- Plans, implements, directs and monitors care coordination programs, services, policies, organizational wide initiatives and improvements that foster, support and advance family center integrated care services that are seen to result in goals set around patient satisfaction, clinical outcomes and utilization management.
Patient-Centered Care:
- Ensure patient care plans are individualized and integrated, addressing the medical, psychosocial, and spiritual needs of pediatric patients and their families.
- Promote patient and family engagement/partnership in care decisions and goal setting through shared decision-making processes.
- Address barriers to care by coordinating resources, systems and services, particularly for at-risk or underserved pediatric populations.
Collaboration and Partnerships:
- Build and maintain relationships with external providers, community organizations, and payers to enhance care coordination services.
- Partner with medical and surgical departments, primary care, and specialty services to enhance communication and ensure a comprehensive approach to patient care.
- Serve as the liaison between clinical and operational teams to optimize care pathways and resource utilization.
- Collaborate with organizational leaders to develop strategic and operational initiatives that support health literacy and children and families understanding and knowledge of health and illnesses throughout the continuum of care.
Quality Improvement and Compliance:
- Lead quality improvement initiatives/best practices aimed at enhancing the delivery and outcomes of care coordination services.
- Ensure compliance with all relevant regulatory requirements and accreditation standards, including those from The Joint Commission, CMS, and other governing bodies.
- Monitor key performance indicators, reporting metrics on care coordination efficiency, patient outcomes, and cost-effectiveness.
Data Management and Reporting:
- Oversee the collection and analysis of data related to care coordination, patient outcomes, and resource utilization.
- Use data-driven insights to identify opportunities for improvement in care coordination practices and patient outcomes.
- Provide regular reports to senior leadership and medical team leaders on program performance, challenges, and achievements.
Accountable for departmental financial management, developing budget recommendations and monitoring budgets, ensuring alignment with organizational financial goals.
Education and Training:
- Provide training and development opportunities for care coordination staff to ensure best practices are implemented.
- Promote continuous learning and professional growth within the department.
- Ensure staff are up-to-date on the latest pediatric care coordination practices and regulations
Education Qualifications - Bachelor's Degree in healthcare related field (i.e. nursing, social work, child life) is required
- Master's Degree from an accredited college or university in directly related health care field required
Experience Qualifications - Minimum five (5) years of progressive program development, implementation, evaluation and sustainability management experience in a pediatric environment is required
- Minimum of three (3) years of operational leadership/supervisory experience is required
Skills and Abilities - Knowledge of hospital operations, billing, collection and reimbursement requirements and standard practices.
- Ability to review, interpret and implement managed care contracts with third-party including governmental payers.

Licenses and Certifications - LCSW- Licensed Clinical Social Worker Upon Hire Required or
- RN - Registered Nurse - State Licensure and/or Compact State Licensure Upon Hire Required and
- Certification in case management, utilization management or other related field Preferred
