Director Acute Case Management & Social Services:

Full-time, Days
40 FTEs
Seeking an experienced Director of Case Management & Social Services to drive quality improvement and program outcomes.

Qualifications:
  • Candidates must possess 5 or more yeares progressive leadership experience in Acute Care Case Management
  • RN and/or Licensed Social Worker required
  • Candidates must have 5-10 years Inpatient Acute Care Case Management experience
  • Certified Case Manager (CCM) required
  • Proven success in a large medical center managing key stakeholders, throughput, and driving strategy
  • ANCC Magnet recognized certification required
