Job Title: Licensed Clinical Social Worker (LCSW)
Location: Washington, DC 20037
Shift: Monday - Friday 8:30 AM - 5:00 PM Key Responsibilities:
Provide social work services in accordance with licensure and training
Coordinate and facilitate patient services throughout rehab admission
Lead discharge planning and care coordination
Offer counseling and planning support for patients and families.
Adapt to new tasks and changing patient needs Qualifications:
Must live 100 miles away
Licensure: Active LICSW and DC Social Work License required
Experience: Minimum 1 year of hospital case management/discharge planning experience
Systems: Knowledge of Cerner EMR preferred
