Summary: The Diabetes Nurse Navigator is responsible for clinical guidance, education and coordination of care for high-risk patients with diabetes and their caregivers with a goal of promoting better health outcomes. Primary focus will be on patients and their caregivers with new diabetes diagnoses, and those who return for emergent and/or inpatient hospital care with signs of uncontrolled diabetes or complications of diabetes. This position collaborates with providers, patients and families, and hospital teammates to accomplish positive results. The Diabetes Nurse Navigator will work to identify and eliminate barriers to care by guiding, educating and navigating patients to receive necessary health care monitoring and resources. The position will be charged with building professional relationships with the patient and their families, local and regional healthcare providers and vendors to ensure smooth transitions and timely access to care for patients. Responsibilities: 1. Patient Education - educates patients and their families regarding care plans and resources and to facilitate shared decision making. Continues education and communication with diabetes patients/families 1:1 and in group settings. Coordinates and participates in diabetes management classes. 2. Facilitator - works with patients/families and appropriate healthcare providers to schedule needed appointments, tests, diabetes management classes, procedures, discharge planning, and follow-up. Also works with the patient to promote goal setting and self-management care practices. Facilitates and assists in community screening and education events. 3. Collaborator - Works with internal/external medical providers, health care team members and patients to reach goals and provide excellent quality of care through care coordination. Ensures that there is continuity of communication between a patient's various care providers. Follows up with patients during and post hospitalization. Participates in nursing, hospital and medical staff committees as appointed or appropriate. 4. Patient Experience - navigates the patient and family through all aspects of diabetes management to answer questions, decrease anxiety and minimize barriers to care. 5. Knowledge Base - develops an in-depth knowledge regarding evidence-based practice and best practices for this patient population. Identifies educational opportunities throughout the organization, and develops and leads educational events, as appropriate. Serves as a resource within the hospital. Keeps current in trends and developments in diabetes care practices. 6. Results Driven - leads the organization to set goals related to this patient population. Leads the organization in identifying opportunities and developing appropriate action plans. Collects and analyzes on-going data regarding outcomes of the Diabetes Management Program. Submits information to regulatory agencies as requested. Fosters a culture of safe and compassionate care. Leads the organization to achieve recognition for excellence in diabetes management. 7. Community Resource Link - connects patients and families to resources, providers and services within the community to deliver a seamless model of timely access and care benefiting patients, participating physicians, and family members. WAYNE Other information: Education Bachelor's Degree in Nursing preferred. Licensure/Certification Current NC license as a Registered Nurse is required. Diabetes Educator Certification or other relevant certification preferred. Experience Minimum of three (3) years of experience in nursing that includes the care of diabetes patients. Knowledge, Skills and Abilities - Knowledgeable of tools and resources available for diabetes management. - Excellent interpersonal and customer service skills, with the ability to build collaborative relationships with providers, teammates, patients, their caregivers. - Ability to read, write and communicate effectively in English. - Proficient with MS Office and able to learn new software rapidly. Valid NC Driver's License: No If driving a Wayne UNC Vehicle, must be 21 years old and MVR must be approved by Risk Management.
