PanCare Health is seeking a compassionate and dedicated
Diabetes Educator to join our team. In this role, you will work closely with patients to provide essential education on managing diabetes, improving health outcomes, and adopting healthier lifestyles. As a Diabetes Educator, you will collaborate with healthcare teams to deliver individualized care and empower patients with the knowledge and resources needed to effectively manage their condition. Responsibilities- Conduct DSME/T program participant assessments using the AADE7 framework, in a collaborative and ongoing manner.
- Collaboratively develop educational goals, learning objectives, and a plan for educational content and teaching methods with DSME/T participants.
- Provide educational interventions utilizing primarily interactive, skill-based training methods to maximize engagement and participant learning.
- Develop individualized follow-up plans for each participant.
- Evaluate the effectiveness of educational services by measuring the attainment of learning objectives.
- Conduct follow-up assessments upon completion of DSME/T services using outcome measures from the AADE7 Continuum of Outcomes Framework.
- Document assessment data, educational plans, services provided, and evaluation results in participants' educational records.
- Collaborate with team members and communicate relevant participant information as needed.
- Identify when a participant's needs exceed the instructor's expertise and refer to other healthcare providers for additional services.
- Communicate relevant participant information to the primary care provider.
- Participate in the development of training materials and contribute to continuous quality improvement processes for the DSME/T program.
- Regularly assess performance to identify strengths and areas for growth and create a plan for professional development.
- Participate in peer reviews to evaluate the performance of other instructional staff.
- Maintain 15 hours of continuing education annually specific to diabetes and behavior change strategies.
- Uphold National Standards for DSMES service design, delivery, evaluation, and continuous quality improvement.
- Oversee effective implementation, evaluation, and tracking of DSMES service outcomes.
- Ensure compliance with accreditation requirements and act as the main point of contact for accreditation surveys.
- Stay informed on emerging evidence, advances in education strategies, technology, pharmacotherapeutics, and delivery methods relevant to the population served.
- Engage in value-based care initiatives and analyze data to improve service outcomes.
Required Skills/Abilities- In-depth knowledge of current diabetes treatment management.
- Ability to lead and effectively manage individual and group classes.
- Ability to develop collaborative, therapeutic relationships with individuals.
- Basic computer skills, including proficiency with the Internet and email.
Education/Experience- Registered Nurse (RN) with active licensure, Certified Diabetes Educator (CDE), Certified Nutritionist, or Pharmacist with specialization in Diabetes.
- Experience in diabetes education or a related healthcare field is required.
- Knowledge of the AADE7 framework and DSME/T programs is required.
Physical Demands- Regularly required to stand, walk, use hands for handling or feeling, reach with arms, and talk or hear.
- Frequently required to sit and occasionally stoop, kneel, or crouch.
- Must occasionally lift and/or move up to 40 pounds.
- Specific vision requirements: close vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
- Must maintain a professional and calm demeanor when interacting with patients, including children.
