Five-Day Work Week - Sign-On Bonus: $30,000 total - $15,000 upon signing - $15,000 one year after the signed contract date - Base Salary: $575,000/year - Minimum: 45 patients/day - Additional Incentive: Once net collections exceed $1.5 million in a year, receive 10% of net collections above $1.5 million until year-end2.
