-Board Certification
-ACLS
-Alaska State license
-EMR System: Paper Charting
-Practice Setting: Outpatient, Inpatient
-Hospital/surgery center privileges: Yes
-Estimated Credentialing Timeframe: 45 days
-Trauma Level Facility: 4

Required procedures:
-General Anesthesia
-Adult
-Spinal Anesthesia
-Epidural Anesthesia
-Regional Anesthesia Surgery
-Obstetrical Cases
-Neurosurgical cases: Spine
-Thoracic cases (except open heart)
-Complex orthopedic cases
-Post-op epidural
-Post-op extremity blocks
-Use of x-ray guidance in performance of blocks
-Ventilation management
-Evaluation and management of acute volume/BP issues
