RESPNSIBILITIES
  • Provide outreach and referral development services with ACS PPRS liaisons, other BronxWorks programs, and community based programs
  • Respond to and triage all referral calls
  • Provide intake services for all referrals, including intake interviews and write ups
  • Collect statistical information from Contact Grids, PROMIS and CNNX and other required databases to write and complete reports
  • Collect information directly from Case Managers and Assistant Program Directors necessary to compile required reports
  • Extract data from data sources to identify important trends and problems
  • Train new staff to use computerized data base programs
  • Submit written reports and analysis that accompany data reporting and presentations
  • Present data findings in internal and inter-agency meetings
  • Interact with ACS on an as-needed basis
  • Participate in grant writing projects
  • Provide quality assurance for case management input and charting in client based data systems including Contact Grids, PROMIS and CNNX
  • Maintain familiarity with program resources available on- and off-site
  • Attend CS meeting for CQI purposes to maintain program standards. Must be able to report on these meeting and changing standards.
  • Assist with special projects as assigned including representing agency at interagency events
  • Report to and meet with the Program Director on a regular basis
  • Perform additional duties as assigned
PROGRAM SPECIFIC RESPONSABILITIES
  • Progress notes review (Contact Grid, Promis, Salesforcer and CNNX)
  • Conduct case record reviews and provide reports
  • Case Planner Report, verify active cases within month Tracked Children Report )
  • FASP Due and FTC Report (Verify CP responsibility and FTC timeframe)
  • Supervisor Case Review, Close Case Report, Connections spot check
  • Staff KPI targets (email), KPI Excel Spreadsheet and Powerpoint, KPI Narrative Submit reports to Program Director
  • Maintain intake, case review, group work logs
  • Assist with ordering Program supplies with Program Director.
  • Any other duties, where appropriate
QUALIFICATIONS
  • Bachelors degree in a related field of study is required.
  • Two years of experience in a social service setting.
  • Excellent math and writing skills.
  • Extensive computer skills including a strong knowledge of Microsoft Excel spreadsheets, Microsoft PowerPoint as well as web-based client databases such as Awards, CARES, Rhio, Salute, PSYCHES, Streetsmart, and HMIS.
  • Proficiency in modern business communications including personal computers, electronic mail, voicemail, facsimile and copier equipment
  • Proficiency in verbal and written English required; verbal proficiency in a second language preferred
  • Strong oral communication, time management and organizational skills are necessary.
PHYSICAL REQUIREMENTS
  • Ability to use a computer for prolonged periods.
  • Ability to occasionally lift and/or move up to 10 pounds.
  • Ability to stand, walk, or sit for long periods of time.
  • Ability to climb five flights of stairs, if required to conduct home visits and/or fieldwork.
  • Ability to bend and retrieve objects and/or documents.
  • Ability to travel in the boroughs of New York City and its adjacent counties via public transportation.
  • Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather, if required to conduct home visits and/or field work.
  • Perform other duties assigned by manager.
