Job Summary - Under the direction of the program Director, the transitional care specialist is accountable for facilitating, implementing, and confirming postacute transition plans for CoxHealth s patient population. The position will respond to hospital and community-based service requests from the nursing staff, case managers, social service workers, hospitalists, or private attending physicians' practices.
Job Requirements - Education
- Required: High school diploma or equivalent
- Preferred: Associate's Degree
- Experience
- Preferred: One year hospital and/or healthcare experience
- Skills
- Good communication skills Ability to communicate effectively in written format and/or oral presentations
- Strong analytical & problem solving skills; semi-independent in decision making
- Ability to maintain organization in a changing environment
- Exhibit initiative, responsibility & flexibility
- Must be able to initiate and understand research related to projects
- Learn and appropriately utilize all required computer applications, including but not limited to E-mail, Windows-based or Microsoft programs
- Ability to write effective documentation for processes and procedures.
