Consultant in Medicine for the Elderly and Acute Frailty
This is a new 8PA post based at St Mary's Hospital Campus of Imperial College Healthcare NHS Trust.
Medicine for the Elderly (MfE) services have evolved over recent years at St Mary's Hospital, now featuring a multiprofessional Acute Frailty Team and ED liaison service, alongside 48 acute MFE beds across 3 wards, outpatients, and liaison work with multiple community services in West London. The acute frailty team requires daily consultant input. This new post has been created to ensure year-round acute frailty coverage on admissions wards and in the Emergency Department, and to support the development of Frailty SDEC at the SMH site.
Main duties of the job
Frailty service development is central to the Trust's planning and operations. The addition of this post holder will enable sustained service coverage and facilitate the design and pilot of improvements within the team. The department actively participates in audit and QUIP initiatives and collaborates with the ED frailty lead. The post holder will engage in local and regional working groups and CQUINs, including the Frailty Assessment CQUIN in ED.
The post involves working with 2 consultant colleagues in the frailty service, primarily based in the acute medical unit, providing acute frailty input to admitted medical patients. Supervision and collaboration with emergency department-based Frailty Practitioners (ACPs) are key components. There are also opportunities for involvement in community and virtual ward initiatives, as well as general medicine admissions and on-call shifts. Imperial College supports flexible working; discussions are encouraged.
