About the Role:
Medix is seeking a Community Supports Director to lead and grow our Enhanced Care Management (ECM) and Housing Programs in the Los Angeles region. Reporting directly to the CEO, this individual will oversee day-to-day operations, supervise multidisciplinary staff, and collaborate with county agencies, health plans, and community-based partners to deliver integrated services for vulnerable populations. This position is particularly focused on housing support services and requires strong familiarity with CalAIM initiatives.
Key Responsibilities:
- Lead, supervise, and support staff including Lead Care Managers, Clinical Consultants, Housing Navigators, and Outreach Specialists
- Conduct weekly caseload audits and ensure ethical, compliant documentation within electronic health records
- Oversee development, evaluation, and revision of individualized care plans for ECM clients
- Coordinate with community partners including County Behavioral Health, Housing Authorities, Health Plans, and other local service providers
- Identify staff training needs; deliver or coordinate training programs
- Contribute to budgeting and grant writing, especially for CalAIM expansion opportunities
- Participate in interagency meetings, conferences, and collaborative planning efforts
- Ensure compliance with applicable state/federal laws and internal policies (HIPAA, Medi-Cal, 42 CFR Part 2)
- Occasionally conduct screenings and assist in direct care planning
Qualifications:
- Bachelor's degree in Human Services (e.g., Social Work, Public Health, Psychology, Sociology)
- At least 5 years of relevant clinical/administrative experience in human services or behavioral health
- Minimum 2 years of experience in a supervisory or team lead role
- Extensive knowledge of housing services, local housing authorities, and CalAIM/ECM implementation
- Proficiency in managing programs serving individuals with co-occurring mental health and substance use conditions
Preferred:
- Master's degree in Human Services or related field
- Bilingual (Spanish)
